A 61year old female casual labour by occupation came to Casuality with the chief complaints of loin pain, burning micturition, odeama, abdominal distension.

HISTORY OF PRESENT ILLNESS

Patient was asymptomatic 1year back and started burning micturition, loin pain

HISTORY OF PAST ILLNESS

She is N/k/c/o DM, hypertension, CAD, asthama, epilepsy.

PERSONAL HISTORY

Loss of appetite 

Decreased urine output

Irregular bowel movements

Weakness

Inadequate sleep

FAMILY HISTORY

Not significant 


GENERAL EXAMINATION 


PHYSICAL EXAMINATION 

No pallor 

No icterus

No cyanosis 

No clubbing of fingers

No lymphadenopathy 

Presence of odeama (PITTING TYPE)


VITALS

Temperature: Afebrile

Pulse rate: 82bpm

Bp: 124/76 mm of Hg


SYSTEMIC EXAMINATION

S1, S2 heard

No murmers

RESPIRATORY SYSTEM 

No dyspnoea

No wheezing 

Trachea- central

Breathsounds - vesicular

ABDOMEN

No tenderness

No palpable mass

No briuts

No palpable spleen and liver

Bowel sounds-yes

CNS

Conscious

Coherent

Cooperative

Speech-normal

No neck stiffness.

Sensory system- Normal

Motor system- normal
