A 55 year old male came to the opd for dialysis.
CHIEF COMPLAINTS:
swelling of abdomen and swelling of lower limbs.
HISTORY OF PRESENT ILLNESS:
Patient was asymptomatic 3months back and later he developed swelling of lower limbs, and he came to opd with swelling of abdomen also.
1 month back he came to opd and investigations shown his serum creatinine levels as 5.3mgldl.
He has been on medication ever since.
HISTORY OF PAST ILLNESS:
Not a k/c/o Diabetes mellitus, CAD, ASTHMA, TB, EPILEPSY, Burning micturition.
FAMILY HISTORY:
No significant.
PROVISIONAL DIAGNOSIS:
Chronic renal failure secondary to nephrotic syndrome with HTN.
